Recently, we have hosted patients from the Bahamas, South America, England, Canada and throughout Florida and the US. It is well worth the travel to reap the benefits of top notch medical care.. We understand that it is often inconvenient, time-consuming and expensive for patients to fly to our office for a consultation then fly back again a few weeks later for surgery. So, we have designed a streamlined program to facilitate your experience that will allow you to determine the options that are available to you with ease and understanding. Many of our out of town patients have found this program extremely beneficial and we hope you enjoy the same experience. The out of town Consultation made easy Here's what you'll need to do:
The first step is to call 888.438.3223, or e-mail our Patient Care Consultant, express your interest in receiving expert care with Dr. Clevens and request our New Patient Registration Forms.
Please provide clear, recent close-up photos. Photos should clearly show your area of concern and may be sent via email or regular mail. Dr. Clevens and his staff will contact you via email or telephone to review your images and discuss your concerns. We may offer computerized preview imaging to help you visualize your new look. In consultation with Dr. Clevens, we will assist you in determining which options will help you achieve your desired goals.
Fill out our new patient information forms and return to your completed forms at your convenience to our office via fax or mail.
Once you have decided to proceed, a deposit is required to reserve your procedure date with Dr. Clevens. The remaining balance for your procedure is due prior to the procedure.
Shortly after your procedure is scheduled, you will receive a packet in the mail containing pre-operative instructions and procedure consents. These documents must be read carefully and all instructions followed closely. All consents must be signed, dated, and returned completed prior to your consultation with Dr. Ross A. Clevens. We advise all patients who fly-in for surgery to spend at least the first night at a nearby hotel or stay with a friend or family member. You will be seen frequently and regularly after your surgery by Dr. Clevens and his staff. Please note that your health and safety is of our utmost concern and you should plan to stay in the local area of Melbourne, FL for a period of time after your surgery so that we may ensure your comfortable recovery.
