Q: Which is better: Fillers or Fat Transfers for Cheek Wrinkle Line? I have a wrinkle line that starts from the inside corner of my eye that goes down and across the cheekbone area. A dermatologist recommended using fillers after doing incision in that area. The results were great from the reviews and pictures I've seen. Second consult was with a plastic surgeon who recommended getting fat transfer there. Which would be best for the wrinkle line across the cheekbone?

A: Fat vs Fillers. This is a question I hear often in my practice. As a general rule of thumb, I prefer to use injectable fillers because they are predictable in terms of duration and placement. In addition, there is no need to harvest your own fat and you can fill the area gradually, over time, for a softer, more natural look.

Q: Are Sculptra Injections Permanent?

A: In my experience, Sculptra lasts about 2-3 years if a proper and full correction is achieved with a set of injections. Most patients require a series of 3-4 treatments spaced out at monthly intervals to achieve their desired result. Once this is accomplished, many patients return for an annual touch up and then the result lasts for many years.

With the popularity of the “liquid facelift” that allow patients to restore volume to the face and obtain a more youthful appearance without surgery, many patients are asking for similar options to rejuvenate the appearance of their eyes. This one is being called the “liquid eyelift.” To set the record straight, neither one is a true lift because the results are temporary. However, for those individuals who only need subtle adjustments, the “liquid eyelift” can be a welcome nonsurgical option.

Using injectable fillers to fill in hollow areas, Dr. Clevens can give the appearance of a subtle lift on parts that are sagging. It is important to note, however, that if there is significant sagging, fillers will not correct it. In this instance, you would most likely be a better candidate for surgery. There are pros and cons to each option, which should be explained by a board-certified facial plastic surgeon such as Dr. Clevens. The results from surgery are permanent and more dramatic but require more downtime. And the results from fillers and, in some cases, Botox as well, used in the liquid option require minimal downtime but need to be done regularly (about every 6 months, on average) to maintain the look.
